If your situation has worsened while waiting, use this 30-second checklist to generate a clinical summary for your GP.

The 'Power Words' for Reassessment

When talking to your GP, using specific phrases can help them understand the urgency. These are the triggers that the system responds to.

Imminent Risk of Hospitalisation”

The #1 trigger for an urgent reassessment.

Unsafe Living Environment”

Use if stairs, bathroom, or other home areas are no longer safe.

Carer Fatigue/Breakdown”

Explains that the support system is collapsing.

Functional Decline”

Clinical term for being unable to perform Activities of Daily Living (ADLs).

1. Check the boxes that apply to your situation.

Has there been a fall or injury in the last 14 days?

This demonstrates an immediate physical safety risk.

Is the primary carer at risk of a mental health breakdown?

The main support person is unable to continue, placing both people at risk.

Is the person unable to feed or wash themselves safely?

This shows a significant decline in Activities of Daily Living (ADLs).

2. Generate the PDF and take it to your GP to review, sign, and stamp.

Once your GP has signed the document, call My Aged Care on 1800 200 422.
Say clearly: "I have a GP-signed Clinical Evidence form for a Priority Upgrade and need to submit it for an urgent reassessment."
They will provide instructions on how to send the document to them.
